GMOA FORMS

GMOA Membership Application

Eligible medical officers can apply for membership. Applicants are required to complete the designated application process and submit the duly completed documents through the prescribed channels for verification and approval, in accordance with GMOA regulations.

Exclusive Car Pass

Exclusive Car Pass for GMOA’s members to facilitate authorized access and identification. Eligible GMOA members may apply through the prescribed application process, and the car pass will be issued upon verification of active membership in accordance with GMOA guidelines.

Membership Verification Letter

The Government Medical Officers’ Association (GMOA) has introduced an official Member Verification Letter to facilitate verification of active GMOA membership for professional and administrative purposes.

Doctor Repatriation Program

GMOA supports the timely return of doctors after approved overseas training, ensuring smooth reintegration, recognition of training, and protection of service seniority and entitlements.

Grade Medical officers’ Temporary attachment request form

The Grade Medical Officers’ Temporary Attachment Request Form, issued by the GMOA Grade Medical Officers’ Transfer Subcommittee, has been published on the GMOA website for the convenience of members. Eligible Grade Medical Officers are kindly requested to download, complete, and submit the form in accordance with the guidelines provided.

Special Appeal Request Form

Members are kindly requested to log on to the Human Resource Management Information System (HRMIS) using the details provided below in order to access and submit the Special Appeal Request Form. Please ensure that you use your correct login credentials to avoid any delays in the submission process.

GMOA members can apply for the admission of their children to Year One at designated schools. Please complete the form with accurate details and submit it to the GMOA office for processing. Ensure all supporting documents are attached to avoid delays.

For the convenience of our members, the Government Medical Officers’ Association (GMOA) has published the official Ministry of Health Research Allowance Forms on the GMOA website.
